The number of extension options is dizzying - so much so that the user is forced to remove LESS useful extensions or sacrifice the time saving value that the program brings to the table. I could see either a folder hierarchy or context awareness. The former would be far easier and probably more satisfying, particularly if paired with hotkeys to navigate the structure.

Noted and understood but please bear with me.
it seems to me there already is a certain amount of “context awareness”
what about the possibility of a primary menu, of most used extensions, and one click to popup a secondary menu with lesser used extensions?
